IN view of reported misrepresentations allegedly perpetrated by certain individuals and entities who misuse, abuse, misrepresent and thus usurp the functions of Philhealth as an office, Philhealth Regional Office-10 regional vice president and concurrent area vice president for Mindanao Datu Masiding M. Alonto Jr. issued Advisory No. 2018-006 re: “Issuance of Philhealth ID Cards and Member Data Record is Free.”

Hence, this issue is dedicated to apprise, guide, inform and, most importantly, protect the greater public against the deceitful and onerous designs of such unscrupulous elements.  The entirety of this very important and timely advisory is cited as follows:

“It has come to our attention that there are individuals posing as intermediaries who collect fees in exchange for facilitating the issuance of PhilHealth Identification Cards (IDs), thus PhilHealth Regional Office 10 strictly impose the following rules in the issuance of IDs and MDR.

“Philhealth IDs will be released only to the member upon the presentation of valid identification documents and 1×1 ID picture.  No authorization will be honored for ID issuance.

“The MDR will only be released to PhilHealth members.  In case when the member is admitted in the hospital, a representative may be authorized to request for the MDR upon submission of an authorization letter signed by the member and a copy of the hospital certificate of confinement to our frontline staff.

“The public is hereby informed that we do not charge any fees for the issuance of the PhilHealth ID cards and MDR to legitimate members.  Kindly transact only at the nearest Local Health Insurance Office or to any Philhealth Business Center in your localities.

“Please immediately report any similar unscrupulous activities to our Public Affairs Unit through email: philhealth10pau@gmail.com or to any Philhealth office near you.”

N.B.: Remember, all Philhealth IDs and Member Data Records (MDRs) are free of charge.
